Online appointment requests
Our CareMed visit request form asks for your name, email address, mobile number, service category and preferred visit time. CareMed uses these details to review availability, contact you and manage your request.
Requests are stored in CareMed’s booking system, hosted using Render and Supabase, and reviewed by authorized CareMed reception staff. The form provides a private link for checking your request and asking to reschedule or cancel. Keep this link private: anyone with it can view and request changes to that request. Booking acknowledgments and updates are sent through Resend to the email address you provide. These messages include your request reference and private status link; reception alerts direct authorized staff to sign in. Email delivery may be delayed, so use your private link to check the latest status. The system does not send automatic SMS updates or add appointments to Google Calendar. Visit requests do not subscribe you to marketing.
Use the form for scheduling only. Do not enter symptoms, diagnoses, medical records, test results, identity numbers or payment details. Ask CareMed how to provide any clinical information needed for your care.

Callback requests
The callback form asks for your name and contact number. Your email address, preferred doctor, service or package, preferred date, time preference and brief scheduling note are optional. These details help CareMed respond to your request and arrange your visit.
Submissions are processed through Squarespace and sent to CareMed at hello@caremed.ph. The form does not sign you up for marketing emails.
Use the form for scheduling only. Do not send medical history, test results, identification documents or other clinical information.
General enquiries
The enquiry form asks for your name, email address and question. An enquiry topic is optional. CareMed uses these details to reply to your enquiry.
Submissions are processed through Squarespace and sent to hello@caremed.ph. Your email is associated with the form submission in Squarespace Contacts. The form does not subscribe you to marketing.
Please use general enquiries for service and visit questions. Do not include medical history, test results, identity numbers or confidential documents.

You can also ask about how long enquiry information is kept, who can access it, and how to request its deletion where applicable.
When you choose an email link, your email app opens so you can review and send your message. Keep general messages brief and ask our team how to share clinical documents.
